Abstract
This report sets out the findings from work undertaken between January and April 2014 to support the reduction of unscheduled emergency hospital admissions from care settings in Oxfordshire, Buckinghamshire and Berkshire. Funding to support the learning and development of the workforce to reduce unscheduled admissions has been placed with the Institute of Vocational Learning and Workforce Research at Bucks New University. This report summarises the work undertaken in the first phase (scoping phase), with development, delivery and evaluation phases outlined and expected to be completed by May 2015.
